Social Worker II - Adult Services CAP
Knowledge, Skills, and Abilities: Must be able to conduct home visits. The screenings will address physical, mental, emotional, economic, and social functions. Responsibilities include working with supervisor in obtaining approval for each plan, arranging for services, monitoring service plan as necessary, counsel recipients and family members regarding long term care service choices.
Minimum Education/Experience: Bachelor's degree from an accredited school of social work; OR a four-year degree in a human service field or related curriculum, including at least 15 semester hours in courses related to social work or counseling experience and one year experience in social work or counseling; OR graduation from a four-year college and two years' experience in rehabilitation counseling, pastoral counseling, or a related human service field, providing experience in the techniques of casework, group work, or community organization; or an equivalent combination of training and experience. Valid NC Driver's License and access to a car daily for home visits required.
